What is an “Enhanced Primary Care Membership”?
The Elevate Health Enhanced Primary Care Membership is our way of breaking down the barriers between you and your healthcare. For one low monthly fee, you get direct, unrestricted access to your naturopathic doctor, so you can stop worrying about cost and start getting the health and wellness services you and your family deserve.
What types of medical and wellness services are included in my membership?
It all starts with unlimited visits with your doctor for naturopathic primary care and wellness services. Yes, that means you can see your doctor for medically necessary visits whenever you need to, and you won’t be charged anything beyond your monthly membership fee. Covered medical services include exams, condition management, preventative visits, acute conditions, chronic illnesses, blood draws, consultation, referrals, and coordination of care. And of course, wellness services like medication review, naturopathic counseling, and nutritional analysis and recommendations are also included. We encourage you to read our Service Plan Agreement for more details about what’s included in your membership.
Are there other benefits of membership?
Yes! Beyond unlimited doctor visits and unrestricted access to naturopathic primary care services, we also offer a whole list of additional benefits. As a member, you’ll get a free account on our smartphone/telemedicine platform where you can communicate directly with your doctor, access all your personal health and wellness information, and even have visits via video chat. You’ll get priority scheduling. Plus, you’ll get access to premium member discounts on everything else we offer at Elevate Health, including supplements, labs, and injection therapies.
What is not covered in my membership?
Your membership does not cover any services which are not offered at Elevate Health. It also does not cover certain specialized procedures offered at Elevate Health, such as injection therapies like prolotherapy, neuraltherapy, trigger point injections, and neuro-prolo (although as part of your membership, you’ll receive exclusive member discounts on these services). We encourage you to read our Service Plan Agreement for more details about what’s not included in your membership.
How much does a membership cost?
Monthly membership fees are $97 for adults, $77 for youth (0-17 years), billed at the end of each month. There is also a registration fee of $200 for new patients or $100 for established patients. The registration fee is due when you sign up, and your first monthly membership fee will be charged one month later.
Where do I sign-up?
You can sign up quickly and easily by calling our office at 406-219-3631. You also have the option of waiting until the end of your New Patient visit to decide whether the membership is right for you.
How do I pay for my membership?
Your monthly membership fees are charged once a month through our secure electronic billing platform. You are welcome to pay by bank account, credit card, or American Express.
When will I be charged?
To initiate a membership, you pay the registration fee ($200 for NEW Patients, $100 for CURRENT Patients). However, you will not be charged your monthly membership fee at time of signup. Your first monthly membership fee will be charged one month later. For example, if you signed up as a new adult patient on October 6th, you would pay $200 on October 6th, then $97 on November 6th, then $97 on December 6th, etc.
Will I have access to my membership benefits immediately?
Yes. On the day you sign up for a membership, all of your your membership benefits will become active immediately. If you have an office visit with your doctor on the day you sign up for your membership, the non-member fee for that visit will be waived.
When do I pay for services at Elevate Health not included in my Membership?
When you purchase services or products at Elevate Health which are not included in your membership, such as supplements, specialized injection procedures, and labs, payment will be due at time of service. However, you will receive exclusive member discounts on these products and services.
Can I cancel my membership at any time?
Yes. The last thing we want is for you to feel like you’re locked in. Our memberships are month-to-month, and you can cancel your membership at any time by giving us 30 days notice. After you cancel, you’ll have a 30-day grace period to re-enroll without a registration fee. If you choose to re-enroll after the 30-day grace period elapses, a registration fee of $197 will be due when you re-enroll.
If I cancel my membership, will I still have access to my doctor?
Yes. If you cancel your membership, you can still receive services from your doctor on a pay-per-service basis.
How often can I make an appointment with my doctor?
As often as you need them! As a member, you are entitled to unlimited medically necessary appointments with your doctor for naturopathic primary care and wellness services. We want you to make those appointments. One of the main reasons we’ve adopted this model is to make it easier and more affordable for you to follow up more regularly, get the care you need, and achieve higher levels of health and wellness. We encourage you to read our Service Plan Agreement for more details about what types of visits are included in your membership.
Does my membership include visits with all Elevate Health providers?
Is this an insurance plan?
No. Your membership is not insurance and Elevate Health is not an insurance company. We only provide the services detailed in our Service Plan Agreement and do not pay you money or reimburse you for any costs or charges you may incur. The membership fees do not apply towards any health insurance plan deductible. The membership does not by itself fulfill the personal health insurance mandate under the Affordable Care Act. We recommend that all members consider obtaining some form of health insurance depending on their individual healthcare needs.
