How do I schedule an appointment?

To schedule a new patient visit, call the office at 406-219-3631!  We are happy to answer any questions you have and get you scheduled with the doctor who best fits your needs.

What is the Free 10-Minute Phone Consult?

Not sure if our office is the right fit for you? Book a free 10-minute phone consult with one of our doctors! These consults provide an opportunity to ask questions about Naturopathic Medicine and find out what to expect when working with us.

Do you accept my insurance?

At Elevate Health we do not contract or bill insurance for visits.  Payment is due at time of service for all visits. Choosing to not contract with insurance companies allows for longer visit times and more focused attention for our patients.

If you would like to submit a claim with your insurance, we are happy to provide you with all necessary billing information. We are happy to help you with insurance matters, but it is up to you to understand your insurance coverage.

Do you accept Health Savings Account (HSA) and Flex Spending Accounts (FSA) for payment?

As a medical office we are able to accept both HSAs and FSAs as payment.  It is the patient’s responsibility to understand what is specifically allowed under their specific HSA or FSA.

How much will my first visit cost?

The cost for the New Patient Visit is $250. Many new patients utilize our  Membership program which discounts the New Patient Visit to $200. The Membership also includes discounted labs, supplements, procedures, and convenient smartphone access to your provider.  It is currently available for patients working with Dr. Bacon, Dr. Lampers, Dr. Brennan, and Dr. Wang.

Your doctor may also recommend lab work, supplements, or procedures, which are additional costs to the visit fee.  If you would like information on the cost, just call the office and we can give you an estimate.

What do I bring to my first appointment?

Once you are scheduled for a new patient visit, you will be sent an email confirming this appointment.  The email will contain a link to New Patient Paperwork for you to fill out electronically prior to your visit.  Completing this paperwork and answering ALL questions is the most useful way you can prepare for your first visit.

In addition to completing this new patient paperwork you may also want to bring:

  • A list of the questions you have for your doctor.
  • A medical timeline if you feel that it would be helpful.
  • Any records you have such as past medical records, labs, or imaging.  (Please feel free to call us at 406-219-3631 to discuss how to have past records sent to our office.)

I want my doctor to look at my previous medical records, how do I get them to you?

Once you are scheduled for a new patient visit with one of our providers you can complete a records request with our office and we will request those records to be sent to our office, or you can contact your other providers directly and have them send the records to us. Our fax number is 406-760-1809. If you already have your medical records, contact our office at 406-219-3631 for the best way to get them to us.

Can a Naturopathic Doctor prescribe my medication(s)?

Naturopathic Doctors can prescribe most common medications. The Naturopathic Formulary is established by the state in which the provider is practicing. We are always happy to check on this for you ahead of booking your first appointment if you have specific medication needs.

Which doctor at Elevate Health is the best fit for me?

Our Patient Care Coordinators are experts on our doctors’ specialties, training, professional experience, temperaments, and musical preferences. The Patient Care Coordinators can recommend a doctor for you based on the information you give them and either schedule a New Patient Visit or book a Free 10 Minute Consultation if you still are not sure. We know that choosing a healthcare provider is a big decision and we are happy to help you find the right fit.

What if I have an emergency?

If you experience a health emergency or urgent care issue, please call 9-1-1 or visit your closest urgent care facility or emergency department. 

Elevate Health is not an urgent care facility, and we do not have an after-hours paging service.

Do you treat the condition and symptoms that I have?

We have created a list of many of the symptoms/illnesses/conditions (add link to What We Treat page) that you can review, or you can call our office and one of our Patient Care Coordinators can assist you. They may recommend a Free 10 Minute Consultation with one of our providers so you can get more information directly from a doctor.

Can I purchase supplements from your office?

Established patients are able to purchase supplements at the office or through our convenient online dispensary (Fullscript). We carry many of our most commonly used physician-grade supplements in the office, but if our stock is low or we do not carry a specific supplement that the doctor is recommending for you, we are happy to special order those products into the office for you.

Do you treat my health concern(s)?

Our doctors treat and offer support for a wide array of health conditions and concerns.  To see if we support your specific concerns we recommend you see our Areas of Expertise Page.  If you aren’t finding what you are looking for feel free to call us at 406-219-3631 and one of our patient care coordinators can get you more information.

What are the office's current COVID policies?

Keeping our patients safe is at the forefront of our work at Elevate Health.  We have many patients who are immune compromised or are medically vulnerable.  To maintain safety in our office with the current COVID-19 crisis we have the following protocols:

  • Rooms and equipment are cleaned appropriately between visits.
  • Telehealth visits are available for patients provided the visit can be conducted without an in-person component.
  • Contactless pick up for supplements and other things patient need to pick up from the office is available.
  • We request that all patients who are currently contagious with COVID-19 not come into the office.  We are happy to either reschedule your appointment or switch your visit to telehealth.

With this ever changing situation we review our policies regularly and change as necessary.  We are happy to answer any questions you may have concerning these policies.
